The shoulder is designed for mobility, which also means many structures contribute to control. Pain while reaching into a cupboard or pressing a weight overhead does not reveal one diagnosis by itself.
Look at the whole movement
A clinician may observe the neck, upper back, shoulder blade and arm, then compare active and assisted movement. Strength, recent workload and the exact painful range can provide useful clues.
Adjust rather than abandon activity
Temporarily reduce the weight, range or repetition that provokes symptoms. Maintain comfortable activity elsewhere and rebuild the specific overhead demand gradually when appropriate. Complete rest can reduce capacity.
Do not ignore major changes
Prompt assessment is sensible after a fall, visible deformity, sudden inability to raise the arm, marked weakness, fever or severe unexplained night pain. These features need a medical evaluation rather than a generic exercise plan.
